Request form to access your personal information

Use this form to request access to your personal information under Australian Privacy Principle 12 (Privacy Act 1988 (Cth). We may need to check your identity before we provide any personal information to you.

We will respond to your request within 30 days.

If your request involves a large amount of information or transcriptions of phone calls, we may need to charge a fee to account for our time and resources. We will let you know if there are any charges and provide you with an opportunity to respond and reduce the size of your request before we continue. We will not share personal details about other people, including TIO staff or telecommunications company representatives. This information will be removed from any personal information that is provided to you, to protect their privacy in accordance with the Privacy Act.
